Instructions:

2. All sections of the form MUST be complete. Provide complete details as requested.
3. All nominations must be received by December 1, 2007.
4. Section I and II must be completed in their entirety to be considered. Again, all fields are required.
5. Comments should be concise and to the point. All entry blanks in Section III allow a maximum of 250 words per question.
6. Provide as much supporting documentation as you feel necessary to give the selection committee a good indication of the nominee’s talents and attributes.

1. In your own words, why are you nominating this person for Technician of the Year?
2. List formal training since January 2002, including college, manufacturer supplied training, maintenance and repair seminars, and training courses by other maintenance organizations.
3. In what ways has this technician contributed to the betterment of his/her coworkers and the maintenance department?
4. List innovations and improvements in repair techniques by the nominee since January 2002. Include service tooling devised to reduce repair time or improve safety, innovations in utilization of existing tooling to improve repair efficiencies, etc.
5. List all organizations the nominee is a member of and accomplishments within the organization that makes him/her a more professional technician/employee.
6. List innovations/improvements in troubleshooting and diagnostic techniques since January 2002. Include new techniques devised to reduce diagnostic time or improve completeness of diagnostic efforts. Include any innovation in tooling to improve diagnostic capabilities.
7. List those characteristics or traits that materially increase the professionalism of the nominee. Please include special awards or recognition related to job performance.
